📝 Key Sections in an Interview Application Form

The form ensures the company collects crucial information required for HR record-keeping and regulatory compliance, regardless of how the candidate's resume was formatted.

  1. Personal & Contact Information:
    • Full legal name, current address, phone number, email.
    • Emergency contact information.
    • Often asks about eligibility to work in the country (e.g., citizenship or visa status).
  2. Educational History:
    • Institutions attended (starting with the highest degree).
    • Dates of attendance and graduation.
    • Degrees or certifications obtained.
    • Specific questions about GPA or relevant coursework, if applicable.
  3. Employment History (Verifiable):
    • A complete list of past employers, usually in reverse chronological order.
    • Precise start and end dates (MM/YYYY).
    • Job title, supervisor's name, and contact phone number for each.

  6. Legal & Authorization:
    • A mandatory Attestation Clause where the applicant affirms that all information provided is true and complete.
    • Authorization for the company to verify all information, perform background checks, and conduct drug testing (if applicable).
    • An agreement acknowledging the company's employment policies (e.g., At-Will status).
